We currently have a vacancy for a Home from Hospital Co-ordinator in the East Riding area
Carers Plus Yorkshire is a well-established charity of over 25 years, providing information, advice and support to unpaid family Carers and the wider community across multiple districts of North Yorkshire including Scarborough, Whitby, Ryedale, Hambleton and Richmondshire.
We are delighted to be able to advertise a new opportunity to join our busy Home from Hospital team.
We are seeking a:
Home from Hospital Co-ordinator (22.5 hours per week)
On a fixed term contract for 12 months in the first instance
Salary: £20,304.86 pro rata based on a FTE of 37.5hrs
The postholder will be helping to expand the established HfH team in Bridlington Their role will be to support patients at their point of discharge; helping them to maintain their independence by providing a range of short-term support within the home. (No personal care involved).
You will need to be experienced in completing initial assessments and support planning. You will be required to link with local hospitals to develop and promote the service as part of the team. Knowledge and experience of supporting vulnerable adults would be desirable. Good communication skills will be required. You will have experience of working with people in their own homes and in the community.
Flexibility is a requirement of the role – some weekend, evening work and Bank Holidays will be expected. Own transport essential.
