Home from Hospital Co-Ordinator
I’ve been a Home from Hospital Coordinator for almost seven years. Prior to this, I was a social worker, working predominantly in hospital settings. It’s a pleasure and a privilege to be able to offer information and support to people returning home following a hospital admission. Every day is different and it’s a role that I really enjoy.
