Salary: £21,049 (pro rata of £26,311 Fte)
Hours: Part-time 30 hpw (A job share would be considered, if this is your preference please indicate this on your application form and how many hours up to 30 p/w you would like to work)
Contract: Fixed-term for 9-months (Starting 5th January 2026)

Carers Plus Yorkshire is a well-established charity of over 30 years, providing information, advice and support to unpaid family Carers and the wider community across multiple districts of North Yorkshire including Scarborough, Whitby, Ryedale, Hambleton and Richmondshire.
The Role:
We are seeking a Home from Hospital Co-ordinator to work across Scarborough, Whitby and Ryedale as required.
The role is to support patients at their point of discharge from hospital, helping them to maintain their independence by providing a range of short-term support within the home. (No personal care involved).
Experience in initial assessments and support planning is desired but full training will be provided. You will be required to link with hospitals to develop and promote the service, therefore good communication skills are essential. Experience of working with people in their own homes and in the community is beneficial. A proactive and positive approach to this role to ensure outcomes are achieved and clients’ needs are met.
Own transport is essential due to visiting clients in their homes, and we provide a generous mileage allowance that covers your fuel from base of work to client visits.
