Salary: £31,940-£34,000 FTE (Salary range is dependent on experience)

Base: Carers Plus Yorkshire, split between Snainton (Scarborough) office and Northallerton office and Bridlington (travel expenses will be paid)

Carers Plus Yorkshire is a well-established charity of over 28 years, providing information, advice and support to unpaid family Carers and the wider community across multiple districts of North Yorkshire including Scarborough, Whitby, Ryedale, Hambleton and Richmondshire.
The Role:
This unique and exciting opportunity has arisen due to the current Manager retiring in March 2025. Home from Hospital is a FREE service helping patients make a seamless transition from hospital to home by providing a variety of help and support, as well as giving reassurance and building confidence. The service is available for patients living in North Yorkshire or East Riding of Yorkshire.
The service aims to support patients at their point of discharge from hospital; helping them to maintain their independence by providing a range of short-term support within the home. (No personal care involved).
Working across the full geography of Carers Plus Yorkshire, the Home from Hospital (HfH) Manager will take responsibility for the operational day-to-day running of the Home from Hospital service including full line management of the HfH Coordinator team (all localities) and will support them to deliver the service operationally. The Manager will ensure the service is delivered in full and in line with contractual requirements.
The successful candidate will have exceptional knowledge of services and provisions in the surrounding areas which the HfH service will collaboratively work with. You will need to demonstrate over 3 years’ experience in a managerial role (or similar) that is relevant to the requirements and skills of this position.
There will an expectation for hybrid working, with a regular presence at Head Office in Snainton, plus consistent working from our office bases in Northallerton and Bridlington. Flexibility is a requirement of the role – some weekend, evening work and Bank Holidays will be expected. Own transport essential.
